PMH Notice Regarding Medicare Program

NOTICE

Pulaski Memorial Hospital Hospice will no longer participate in the Medicare program. (Title XVII of the Social Security Act) effective October 31, 2016. The agreement between Pulaski Memorial Hospital Hospice and the Secretary of Health and Human Services will be terminated on October 31, 2016, in accordance with the with the provisions of the Social Security Act.

No payment will be made by the Medicare program under this agreement for covered services furnished to patients who are admitted on or after October 31, 2016.
