The nature of the suit against you is: COMPLAINT TO FORECLOSE AND REFORM MORTGAGE, on certain real estate located in Pulaski County, Indiana, described more particularly as follows:

Lot Numbered (4) as shown on the recorded plat of Joseph B. Agnew Addition to the Town of Winamac, Pulaski County, Indiana.

More commonly known as: 109 EAST 11TH STREET, WINAMAC IN 46996

This summons by publication is specifically directed to the following named defendants whose whereabouts are unknown: Matthew Kopka.

This summons by publication is also directed to the following named defendants: Matthew Kopka, State Of Indiana, Department Of Revenue And John Schmicker
